The rigorous selection process includes developing a pool of qualified candidates based on nominations from other professionals in the medical field. The awarding agency also researches these candidates by searching medical databases, online sources, and publications. Doctors who have attained certain credentials, honors or professional achievements are also taken into consideration. This process also includes physicians in less visible treatment areas.

The award is also given to highly specialized medical physicians who perform the latest procedures, such as eyelid lift (blepharoplasty), ear tuck (otoplasty), nose reconstruction (rhinoplasty), face lift (rhytidectomy), and chin augmentation (genioplasty).

Further research on these narrowed candidates is conducted and includes how many years of experience they have, fellowships, certifications, publications, lectures, and more. Nominees are then grouped into specialties, and those with the highest scores from the above steps are reviewed and evaluated again.

Only the highest scoring doctors from these lists are awarded the title of Super Doctor and usually includes the top 5% of the physicians within the respective state or region.
